For purposes of processing the BPI Payroll Account opening only, I hereby authorize and consent to TTEC Customer Care Management Philippines, Inc./TTEC Customer Care Management Philippines Branch / TTEC India Customer Solution Philippines Branch (collectively "TTEC") to obtain and submit to BPI the information listed in the form.

If you opted to convert your existing savings account to TeleTech payroll, it will be requested to BPI for payroll account tagging and conversion. Upon separation with the company, your account will revert as Savings Account and same conditions when you originally opened the savings account with BPI will apply.
